1. Average 95% or higher on Safety, average 85% or higher in Respect and Responsibility on point sheet for 4 weeks
a. To include completing classroom assignments and work
b. Staying safe at school and bus
2. Team meeting with district and IEP team to plan transition. Team includes parent/guardian, home district rep, Fir Grove classroom teacher and team.
a. Student makes presentation to IEP team about skills learned, and why the student is ready to transition back to district. Presentation could be but not limited to; a powerpoint/prezi, essay, oral presentation, visual representation. Student is able to bring a youth advocate or peer support for presentation.
3. Transition looks different for each student- but will include
a. Partial days at Fir Grove and district school, or
b. Alternating days at Fir Grove and district school- depending on travel time
c. Length of time for split/alternating days determined at transition meeting- will vary per student
4. Full time at district school
